The New York Workers’ Compensation Board has simplified the claim filing process for disability benefits and will request more information earlier in the life of claims.
Two forms — DB-450, Notice and Proof of Claim for Disability Benefits, and DB-300, Notice of Proof of Claim for Disability Benefits of Unemployed Claimants — have been combined into new Form DB-450, Notice and Proof of Claim for Disability Benefits.
