The Nevada Workers’ Compensation Section posted a new form that injured workers must sign to release medical and other information when submitting a claim notice electronically.
The WCS said that when an injured worker uses an electronic signature on a Form C-4, “Claim for Compensation/Report of Initial Treatment,” the treating provider must also require the worker to complete a Form C-4A, “Release of Medical and Other Information.”
