Lumbar Fusion Surgery Rules Released

Workforce Safety & Insurance has discontinued the requirement for a provider to submit the Pain Questionnaire and Surgery Expectation Questionnaire forms as part of the prior authorization process for elective lumbar fusion. WSI initially developed the questionnaires as adjuncts to the Lumbar Fusion Surgery Guidelines.
