Facts: Scott Cody worked for the Ark Glass & Glazing Corp. as a window glazer. He injured his back while at work in April 1999, and in August 2001 he began receiving benefits for a 75% permanent partial disability.
In November 2012, Cody settled the indemnity portion of the claim. He returned to work full time in June 2014.
