WCS Posts Revised Form to Request Rating Exam

The Nevada Workers’ Compensation Section posted to its website a revised form that insurance companies are required to use for requesting an evaluation to determine the extent of permanent impairment in an injured worker.

Carriers are required to file a form D-35, Request for a Rotating Physician or Chiropractor, within 30 days of receiving a report indicating an injured worker may have suffered a permanent disability, in order to set up an impairment exam.

The form, which the WCS said was revised as part of the process “to move forward in automating our systems,” becomes effective today.
