Oregon’s Workers’ Compensation Division announced that carriers and self-insured employers will have to request approval to use modified claim forms by May 1.
The division said payers will need authorization to revise 16 forms, including those to report injuries or illnesses, applications for approval of lump-sum settlements, and informational packets.
