New Industrial Commission Rule Requires a Form that Isn't Available

North Carolina Industrial Commission rules that took effect Nov. 1 require medical providers to file Commission Form 25PR to request preauthorization for medical treatment. Problem is, the form doesn't exist yet, the commission confirmed Friday. Commission Rule 04 NCAC 10A.1001(k) holds that requests for preauthorization for surgery and inpatient treatment must be submitted on Industrial Commission Form 25PR.
