The New York Workers' Compensation Board kicked a Middletown psychiatrist out of the system Monday after he lost his license to practice medicine in the state last week.
Stanley Giudici signed a document last month agreeing to surrender his license after the New York State Board for Professional Medical Conduct accused him of improperly prescribing medications and failing to maintain adequate records.
The document ordered him to surrender his license last Thursday.
On Monday, WCB Chairman Kenneth Munnelly suspended Giudici's license to treat injured workers.
