§ 59A-29.004 Occasional Health Care Providers.
| (1) An occasional health care provider shall meet the following requirements in order to provide services under Section 440.13, F.S.:
(a) Treat twelve or fewer injured employees annually in the calendar year before and after the date that the health care provider submits application form AHCA Form 3160-0020 to the Agency. (b) Meet the requirements of paragraphs 59A-29.002(1)(a) and (b), F.A.C. Occasional health care providers are not required to meet the requirements of paragraph 59A-29.002(1)(c), F.A.C., but are encouraged by the Agency to do so. Anesthesiologists, radiologists, and pathologists licensed under Chapter 458 or 459, F.S., and who do not and will not submit preliminary notices of injury or treatment plans to a carrier under Section 440.13(4)(a), F.S., are not required to meet the requirements of paragraph 59A-29.002(1)(c), F.A.C., but are encouraged by the Agency to do so. (c) Not be ineligible for certification pursuant to subsection 59A-29.002(2), F.A.C. (d) Not be decertified pursuant to subsection 59A-29.006(5), F.A.C. (2) Occasional health care providers shall comply with all provisions of Chapter 440, F.S., and all administrative rules adopted by the Agency. Specific Authority 440.13(3)(a), 440.591 FS.
