§ 59A-29.007 Recertification Process.

(1) When the Agency suspends a health care provider, the health care provider may apply for recertification upon completion of the following:

(a) Completion of the period of suspension; and (b) Submission of a current application to the Agency; and (c) Completion of a repeat certification training course.

(2) When a health care provider's license has been revoked, suspended, or the health care provider has been placed on probation, by the Agency for Health Care Administration, proof of reinstatement shall be submitted with the health care provider's request for recertification.
