§ 6A-22.009 Employee Responsibilities
| Upon approval of Department sponsored reemployment services,the
injured employee and Department staff shall sign and date a Department and student agreement for sponsorship of training and education form DWC-24,which is incorporated by reference in Rule 6A-22.011,F.A.C.
