§ 5210.0567 CAPTION; CASE TITLE.

Subpart 1. Notice of contest. Pleadings after receipt of

a notice of contest must be titled: Commissioner of the Minnesota Department of Labor and Industry, Complainant v. (Name of contestant), Respondent


Subp. 2. Third party interests. Cases in which a third party interest has been exercised must be titled: Commissioner of the Minnesota Department of Labor and Industry, Complainant v. (Name of employer), Respondent (Name of employee or authorized employee representative), Employee or authorized employee representative


Subp. 3. Placement of titles. The titles listed in subparts 1 and 2 must appear at the left upper portion of the first page of any pleading or document filed, other than an exhibit.


The first page of any pleading or document, other than an exhibit, must show, at the upper right of the page, opposite the title, the docket numbers assigned by the commissioner and the Office of Administrative Hearings, if any.
