§ 5215.0600 RECORD ADDRESS.
| The initial document filed with the board by a party or
intervenor must contain the party or intervenor's name, address, and telephone number. Any change in the party or intervenor's name, address, or telephone number must be communicated promptly in writing to the board, all parties, and intervenors. A party or intervenor who fails to furnish the information required by this part waives the right to notice and service under this chapter.
