The U.S. Occupational Safety and Health Administration issued a notice of safety violations to an Army Reserve facility in Sacramento where a civilian worker was fatally crushed by a utility vehicle cargo box.
The U.S. Army Reserve 63rd Regional Support Command has 15 business days to correct the violations or contest the notice, which was issued Nov. 16.
According to OSHA, the employee was killed on May 23 when the automated lifting mechanism for a utility vehicle cargo box failed and pinned the employee between the bed and the frame.
