The Oregon Workers' Compensation Division posted a revised form for physicians and authorized nurse practitioners to complete when releasing a person to return to work.
Revised Form 3245 includes boxes for providers to check off when a worker is released to regular duty or not released to work at all. It also has a number of boxes for providers to indicate restrictions when a person is released for modified work.
The division said changes to the form should make it easier for providers to use.
