Mich. Releases New Disclosure Questionnaires

The Michigan Workers' Compensation Agency released new disclosure questionnaires on its website. The agency posted two new forms – the work history, work qualifications and training disclosure questionnaire (WC-105A) and the employer disclosure questionnaire (WC-105B). A limited number of forms will be available at hearing sites, the agency said. The agency noted that completion of both forms is voluntary, and that they may be used by magistrates to facilitate the exchange of information among parties.
