Board Releases New Forms for Special Disability Fund Reimbursements

The New York State Workers’ Compensation Board on Friday released new forms that must be used to request reimbursement from the Special Funds Group, effective June 1. Subject No. 046-1063 includes links to the four forms that must be used to request reimbursement for payments made to claimants who had concurrent employment or who suffered previous industrial injuries.
