Division Revises Claim Acceptance Form 3058

The Oregon Workers' Compensation Division has revised its notice of claim acceptance form to reflect rule changes that will take effect on Jan. 1, 2017, according to a bulletin issued by the division on Thursday. Form 3058, "Notice to Worker," is used to comply with rules that require insurers and employers to give notice to employers that they have accepted a claim.
