The Oregon Workers' Compensation Division has revised its notice of claim acceptance form to reflect rule changes that will take effect on Jan. 1, 2017, according to a bulletin issued by the division on Thursday.
Form 3058, "Notice to Worker," is used to comply with rules that require insurers and employers to give notice to employers that they have accepted a claim. It can be used to supplement initial acceptance notices, the division said in its notice.
The division said it is working on a Spanish-language version of the form, 3058s.
