DWC Proposes Revisions to Medical Evaluation Form

The Texas Division of Workers’ Compensation (DWC) has proposed revisions to DWC Form-069, Report of Medical Evaluation.Doctors selected by the DWC, an insurance carrier or a treating doctor must use the form when evaluating permanent impairment or maximum medical improvement of an injured employee.Treating doctors also may use the form to agree or disagree with the other doctors’ findings.The proposed revisions are intended to "provide more clarity and harmony" with provisions in the Texas Administrative Code (28 TAC Section 130.1) regarding certification of maximum medical improve...
