Federal safety inspectors who visited a Houston maintenance shop in August found workers continued to operate machinery without safeguards.
Because the setup could have led to a worker losing a limb, the Occupational Safety and Health Administration fined Machinery Maintenance Rebuilders about $155,000 for the safety violations.
