Board Aims to Simplify Disability Claim Filing Process

The New York Workers’ Compensation Board has simplified the claim filing process for disability benefits and will request more information earlier in the life of claims. Two forms — DB-450, Notice and Proof of Claim for Disability Benefits, and DB-300, Notice of Proof of Claim for Disability Benefits of Unemployed Claimants — have been combined into new Form DB-450, Notice and Proof of Claim for Disability Benefits.
