New Form Makes it Easier to Add Non-Employer Info on Hearing Requests

Michigan has revised its application form for mediation and hearing requests to make it easier to list additional parties involved in the case.

Revised Form WC-104C now includes a checkbox for "add non-employer entity," above the name and address field, the state Department of Licensing and Regulation announced.

"Until this addition, petitioners could add another employer, but found it difficult to simply add another party to the case," the bulletin reads. "Utilizing this new checkbox will allow users to more easily indicate their intentions, and will aid in processing."

Parties requesting mediation or hearing are encouraged to begin using the form immediately.
