California's occupational safety agency has issued five citations and $51,160 in penalties to Aero Pacific Corp. in Placentia after a worker was struck and killed by a moving spindle.
The company failed to train workers on how to control hazardous energy, inspectors found. It also failed to identify and correct machinery hazards in the workplace, according to Cal/OSHA's report.
The worker had been setting up a Viper CNC milling machine when the accident occurred.
Aero Pacific Corp. manufactures airplane parts.
