The U.S. Occupational Safety and Health Administration has issued four citations with fines totaling $62,800 to Gary's Grading and Pipeline Co. of Monroe for allegedly exposing workers to excavation hazards while installing a tie-in pipe between two manhole basins at a site in Gainesville, Ga.The company received two repeat citations with fines totaling $56,000 for allegedly failing to inspect a trench after conditions changed and allegedly exposing workers to engulfment hazards by not having a protective system in place to prevent a trench collapse.The company was cited previously for suc...
