Slew of Amputations Brings $88,200 in Fines Against Envelope Printer

After three workers suffered amputations within four months, the U.S. Occupational Safety and Health Administration proposed $88,200 in penalties against an Ohio envelope printing plant. OSHA said it cited Envelope 1 in Columbiana, Ohio for three willful and four serious violations after a Dec. 15 inspection revealed the company had failed to protect workers from moving machine parts on 26 out of 27 of its production lines.
