After three workers suffered amputations within four months, the U.S. Occupational Safety and Health Administration proposed $88,200 in penalties against an Ohio envelope printing plant.
OSHA said it cited Envelope 1 in Columbiana, Ohio for three willful and four serious violations after a Dec. 15 inspection revealed the company had failed to protect workers from moving machine parts on 26 out of 27 of its production lines.
OSHA said its investigation revealed three amputation accidents last summer:
