QME Placed on Probation for Medical Board Discipline

The Division of Workers' Compensation placed a qualified medical evaluator on probation until 2017 after he was disciplined by the Medical Board of California for negligent treatment provided to four patients. The DWC revoked the QME certification of Dr. John J. Kayvanfar, but stayed the revocation and placed him on probation from Nov. 21, 2014, until Nov. 20, 2017.
