DWC Seeks Comments by May 11 on Required Medical Exam Form

The Texas Division of Workers’ Compensation will receive public comments until May 11 on proposed revisions to the DWC Form-022, Required Medical Examination, Request for Agreement/Request for Order.The form is for insurance carriers requesting an injured employee to attend a required medical examination to be performed by a doctor of the insurance carrier’s choice.
