Window Manufacturer Fined $74K for Amputation Hazards

After a Pennsylvania man's finger was amputated on his first day of work, his employer did not report the injury or fix the associated workplace safety hazards, an Occupational Safety and Health Administration official said. OSHA inspected the Dalton, Pennsylvania, facility of Crystal Window & Door Systems in May. Inspectors had received a complaint that the window manufacturer had not reported an amputation six months earlier.  Inspectors found:  Unguarded machinery. No established energy control program.  Powered industrial trucks operated by untrained employees...
