On Brandon Horine's first day on the job for RP Construction this September, the 42-year-old fell off a roof while removing shingles from a home in Nampa, Idaho. The brain trauma he suffered ended his life a few days later.
The Occupational Safety and Health Administration fined RP Construction $13,971 last week for failing to provide Horine and other employees with necessary training and fall protection equipment.
RP Construction also received a fine for failing to report Horine's hospitalization or death.
