DWC Accepting Comments on Proposed Complaint Form

The Texas Division of Workers' Compensation is accepting comments on a new form that can be used to submit allegations of administrative violations. The proposed DWC Form-XXX, or Workers' Compensation Complaint Form, was mandated by Texas Administrative Code Section 180.2(a), which requires the division to develop and post on its website a "simple, standardized form for filing complaints."
