New Designated Doctor Forms Required Dec. 6

The Texas Division of Workers’ Compensation posted a reminder on Friday that system participants on Dec. 6 must start using two new forms — DWC Form-032 and Form-068 — to comply with updated rules adopted last month.

Form-032 is used to request a designated doctor exam, and Form-068 is used to report the results.

The new forms are mandated by changes to Chapter 127 regulations approved Oct. 15 by Commissioner Cassie Brown that the division hopes will ensure the availability of doctors qualified to evaluate injured employees.
