Manufacturer Fined $107K for Repeat Violations

An Ohio plastics manufacturer faces $107,168 in penalties after the U.S. Occupational Safety and Health Administration found that the company exposed employees to amputation and struck-by hazards at its plant in Carey.
OSHA cited Continental Structural Plastics Inc. for one repeated and three serious violations for failing to adequately train employees on lockout/tagout procedures, for failing to ensure that locking devices were attached to machines as required, and for exposing workers to struck-by hazards from improperly stored materials.
The company was cited for similar violations in 2015.
The citations state that Continental stacked materials in a way that put workers in danger if the piles were to collapse. Also, industrial vehicles operated near shelving units that had damaged legs, indicating that employees were at risk if the shelving were to be struck and collapsed.
The company has 15 business days from receipt of the citations and penalties to request an informal conference with the OSHA area director or contest the findings before the Occupational Safety and Health Review Commission.
