A New York worker who died after getting pulled into the rotating shaft of a coal elevator six months ago was not properly trained, OSHA’s Buffalo office has determined.
The death of the 60-year-old Tonawanda Coke Corp. worker could have been prevented had the company shut down the elevator or locked out its power source, the federal agency said.
The employee has not been named.
