The U.S. Occupational Safety and Health Administration has hit an Ohio ethanol plant with an eye-popping 46 safety violations during three inspections last November, the agency announced on Wednesday.

The company was cited for not having an emergency response plan for hazardous waste operations and for failing to identify flash fire hazards in its ethanol processing area.
