The federal workplace safety agency fined an Illinois demolition company about $150,000 this month for hazards that led to a worker's death in a bridge beam collapse one week before his 48th birthday.
The accident took place in April during construction on the bridge over Interstate 90 in Des Plaines. An overstressed 40-ton beam collapsed because Omega Demolition Corp. had not followed safety protocol to keep pressure off the beam, the Occupational Safety and Health Administration said.
