As he hand-polished a metal cylinder in a factory this winter, a 36-year-old worker became caught in the machine's operating spindle. His injuries led to his death two days later, on St. Patrick's Day.
His employer of two years, Carlson Tool & Manufacturing Corp., of Cedarburg, was penalized this week for violations of machine safety standards.
