Division Issues New Employer-at-Injury Form

The Oregon Workers’ Compensation Division on Friday released a new form to be used by employers seeking reimbursement for transitional work programs under the Employer-at-Injury Program. The revised Form 2360 adds a correction check box to indicate the request is a resubmission of a corrected return request. It also adds a section to request information about concurrent injuries under Oregon Administrative Rules 436-105-0530.
