WCS Updates Claim Forms

The Nevada Workers’ Compensation Section promulgated revised claim notification forms for use on or after Oct. 16. The WCS distributed an updated D-12(a) form for injured workers or employers to request a hearing on a contested claim. The DWC also released an amended form D-30 for employers to notify injured workers when a claim is accepted. Use of the new forms is mandatory, the WCS noted in a statement Tuesday.
