The South Carolina Workers' Compensation Commission has modified its physician's statement form. Form 14B will now include a section clarifying whether future medical treatment is required.
In the notice announcing the modification, the commission reiterated when 14B is necessary:
When a matter is to be resolved at an informal conference.
When a Form 16A (agreement for permanent disability) is submitted.
When a commissioner is considering Final Agreement and Release documents and the claimant does not have a lawyer.
