Three employees of a Southern California medical office were arrested Wednesday on accusations that they submitted fraudulent claims to an insurer for treatment that they claimed they received from their employer, the California Department of Insurance said.
The insurer, which was not identified, paid $540,311 out of $4 million in false claims submitted from 2010 to 2016, the department said.
Three employees of ENT Medical Specialty Group in Diamond Bar were charged:
- Isabel Guizar, 45.
- Erika Lopez, 40.
- Susie Quezada, 50.
The three claimed treatments including nasal surgeries, laryngoscopy biopsies, impaction removals, swallowing dysfunction, nystagmus tests, nasal injections and therapeutic vocal cord injections. They provided the insurance company with handwritten receipts and altered claim forms, and were reimbursed directly.
In addition, one of the suspects, Quezada, has also been accused of embezzling approximately $700,000 from ENT Medical Specialty Group and filing a false tax return.
Guizar and Lopez were booked into West Valley Jail. Quezada was booked into the Los Angeles County Sheriff’s Department Century Station.
The case is being prosecuted by the Los Angeles County District Attorney’s Office.
