Fatal Trench Collapse Leads to $714K in Fines Against Plumbing Company

A plumbing contractor in Blue Springs, Missouri, is facing proposed fines of $714,142 after a worker died in a trench collapse at a residential construction site. Arrow Plumbing LLC was issued citations on June 13 from the federal Occupational Safety and Health Administration following inspections of the work site in Belton, where the 33-year-old employee died on Dec. 15 when a 12-foot trench collapsed.
