A plumbing contractor in Blue Springs, Missouri, is facing proposed fines of $714,142 after a worker died in a trench collapse at a residential construction site.
Arrow Plumbing LLC was issued citations on June 13 from the federal Occupational Safety and Health Administration following inspections of the work site in Belton, where the 33-year-old employee died on Dec. 15 when a 12-foot trench collapsed.
OSHA also found the contractor’s employees working in an unprotected trench at a second inspection site, in Kansas City, in January. No employees were injured there.
