OSHA Fines Plumbing Contractor $274K After Worker Dies in Trench Cave-In

James Rogers, 33, of Winchester, Ohio, died in a trench collapse while working for KRW Plumbing in June. Earlier this month, the federal workplace safety agency fined the company $274,359 after two inspections. Occupational Safety and Health Administration inspectors visited KRW Plumbing worksites in June — after Rogers' death — and October. They found that the company did not have procedures in place to keep cave-ins from happening or keep workers safe in the event of a cave-in.
