The operator of a San Antonio physical therapy clinic is accused of attempting to defraud the federal Office of Workers’ Compensation Programs, an indictment says.
Rafael Enrique Rodriguez, 48, who runs a 210 Workers clinic in San Antonio and another in Utah, allegedly billed for services that were not provided, and used the identities of a doctor and licensed physical therapist without their permission.
