California safety regulators issued fines of $540,890 against an alternative fuel company for violations it says led to a 2011 explosion that seriously injured two workers.Timothy Larson, a veteran Los Angeles city firefighter who was out on disability leave, lost his right arm below the elbow and his right leg below the knee in the Aug. 9, 2011, explosion.
