ICA Publishes Formal Rulemaking Proposal for Treatment Authorization Process

Employers could pay anywhere from $325 to $700 for utilization review under a rulemaking proposal from the Industrial Commission of Arizona to implement a 2012 bill requiring it to adopt state-specific evidence-based treatment guidelines.
