TWCC Revises Forms 41 & 42

The Texas Workers' Compensation Commission has revised forms TWCC-41 and TWCC-42 to make them consistent with the new on-line forms system now in use on TXCOMP. TXCOMP filing has been available for employees, beneficiaries, and their representative to use in lieu of filing paper forms since September 16, 2004. To ensure consistency with the online system, the paper versions of these forms have been modified, TWCC advises.
