A Chemical Safety Board report released Tuesday concluded that "organizational and safety deficiencies at all levels of the BP Corp." led to the March 23, 2005, explosion that killed 15 workers and injured 180 others at BP's Texas City refinery. The event has been called the worst industrial accident in the United States since 1990.
