A report by an independent panel led by former Secretary of State James A. Baker III concluded British oil company BP failed to emphasize process safety at its U.S. refinery operations prior to a 2005 explosion at its Texas City facility which killed 15 people and injured more than 170 others.
The 11-member panel said in a statement summarizing its 300-page report that BP had emphasized personal safety and had reduced personal injury rates but "did not emphasize process safety."
