A North Reading, Mass., contractor faces $115,000 in penalties after an inspector observed workers in deep trenches that had no means of escape if the walls caved in. The U.S. Labor Department's Occupational Safety and Health Administration (OSHA) has proposed hefty fines against Revoli Construction Co. for failure to provide cave-in protection at a Gloucester sewer-line project.
