Commission Revises Action Request Form

The Maryland Workers' Compensation Commission revised form H25R (Request for Action on Filed Issues) to include a certification of service that must be completed. The commission reported that failure to notify opposing counsel prior to the hearing "may result in a penalty or fine to be assessed." The commission asks that old versions of the form be destroyed and that the revised form dated February 2007 be used.
